Chatting to Wellness is a phone call service that brings trained volunteers to seniors while seniors are in their own homes. Chatting Sessions allow isolated seniors to talk, out loud, about what is on their mind. The scheduled chats may occur as often as weekly, or, just as a one-time experience. 

Choose to Move is free and flexible and provides you with the motivation and support to become more active. Choose to Move can help you to integrate activity into your daily routine, meet new friends, and make a positive change! Choose to Move is great for anyone age 65 and older that wants to become active, regardless of ability. 

Self-Management BC

Self-Management programs give people the knowledge, skills, and confidence they need to successfully manage chronic health conditions.

This is a web-based, 6-week program in which groups of people living with a variety of ongoing health conditions participate together.

This is a one-on-one telephone-based coaching program in which Health Coaches connect with participants by telephone, once a week for 30 minutes, for a period of three months.

Participants in this 6-week program receive the materials from the Tool Kit for Active Living with Chronic Conditions Program, and participate in a 30-minute, small group conference call with a Program Leader and other participants.

Includes all tools in the Chronic Disease Self-Management Program (CDSMP), packaged into a one-time mailing. It offers an alternative method for those who cannot or do not wish to attend a CDSMP workshop in person or online.
